Health Net Elect Open Access (HMO)
Health Net Elect Open Access is a health maintenance organization (HMO) plan with a Point-of-Service (POS) provision. The HMO provision requires that the member selects a Primary Care Physician (PCP) from one of the Health Net Participating Physician Groups. The POS component gives you the option of seeking consultations and evaluations from any specialists within the Health Net network without a referral from your PCP. The HMO provision is referred to as Tier 1 and the POS as Tier 2. For detailed information refer to the Health Net Open Access Evidence of Coverage (EOC).

Kaiser Permanente (HMO)
The Kaiser Permanente Health Maintenance Organization (HMO) is available only to employees and their eligible dependents living within the Kaiser Permanente zip code service areas of Los Angeles, Orange, Riverside, San Bernardino, San Diego, Kern and Ventura Counties. Certain outlying zip codes within the County are not eligible for coverage through Kaiser Permanente. Please contact Kaiser Permanente's member service number to verify that you are in an eligible service area. For detailed information refer to the Kaiser Permanente Evidence of Coverage (EOC).

Health Net (PPO)
Health Net PPO is a preferred provider organization. A PPO is a medical plan that offers you a choice between an in-network group of providers who offer their services at discounted rates and out-of-network providers without discounted rates. DeltaCare USA (HMO)

Delta Dental (PPO)
Delta Dental PPO allows you to choose to receive care from a network provider or from an out-of-network provider. It is your choice. You may change between in-network and out-of-network dentists anytime without notifying Delta Dental in advance. With a network dentist, you pay a percent of the dentist’s discounted Delta Dental PPO rates. With an out-of-network dentist, you are subject to pay a higher percentage of Delta Dental PPO’s maximum allowance, plus any additional cost charged by the provider.
