My Health Matters! (MHM) - 2011-12 A New You! Campaign
The County of San Bernardino is committed to providing you with the very best information and resources to support your healthy lifestyle. Be sure to check out My Health Matters!, a personalized and interactive wellness program that can fit into your busy schedule. Whether you are looking to lose weight, quit smoking or just live healthier, this program gives you the tools and encouragement you need to reach your goals.
Click on the links on the right to learn about the various wellness initiatives available to you as part of the 2011-12 A New You! Wellness Campaign that began on September 1, 2011. Many of the initiatives also include drawings for reward cards. There will also be a final drawing for eight $250 reward cards.

Health Risk Assessment – Must be completed between October 1, 2011 and December 31, 2011
The first step on the road to better health is to take the Health Risk Assessment. Answer a series of questions about your health and lifestyle, and you will receive an online personal health plan that reports your current health status and recommendations for improvement.
Complete the questionnaire at Health Net or Kaiser Permanente websites between October 1, 2011 and December 31, 2011 to meet one of the three A New You! requirements to earn a $100 reward card.
